Mayberry Dental Oral Health Maintenance Rewards Program

We are dedicated to helping our patients develop and maintain the highest standard of oral health, and to help further that goal we have established a rewards program for all our patients that maintain a regular bi-annual preventive oral health maintenance schedule in our office.

Here’s how it works:

  1.  Starting at the beginning of 2011, each time you come into the office for your oral health maintenance visit, you receive 50 reward points. Patients recommended for a more frequent schedule, every 3 or 4 months will receive additional points based on the number of maintenance visits per year. The minimum number of maintenance visits per year to qualify for the rewards program is two. Each maintenance visit generates 50 reward points, up to a maximum of 150 points per year.
  2. Reward points can be used in the office towards payment of dental services or products.  Points generated are valued on a dollar per point basis. Although each point can be used toward payment for any office charges, the points themselves are not redeemable for anything other than dental services or adjunctive dental products actually dispensed from the office.  Reward points have no cash value.
  3. Points earned are accumulated over the course of each year and become available to use or transfer 30 days after the second maintenance visit.
  4. Reward points are transferable and can be made available for transfer to family members and friends upon written request of transfer. Rewards points expire upon the death of the doctor, or the reward holder, which hopefully is a long way off.
